SDGVA Belt Test Invites

This form is for SDGVA charter school invitees. Please fill this out if you are planning in your student coming participating in the Free DMA invitational classes from February 23rd - March 28th, and participating in the belt test on March 29th at 10:00 am.

DOB - Students Date of Birth.

Parent Name at the bottom. 

 

Keep in Mind, there is a cost for the belt. 

White Belt w/ Black Stripe - $60

White Belt w/ Yellow Stripe - $70

Yellow Belt - $80
